Use this form to apply for a teaching certificate if you have completed an approved West Virginia Board of Education program as part of the Option 3 pathway (non-college/university).

Convert a Provisional Teaching Certificate to a Professional Teaching Certificate.

Form 20M allows you to be issued a Professional Teaching Certificate if you are a qualifying veteran and meet requirements as per HB 3125 and SB 765. 

The Provisional Certificate is issued to individuals who complete an alternative certification program.

Apply to teach in an area of critical need and shortage in a West Virginia public school district by enrolling in an approved West Virginia Board of Education county-based alternative certification program.

Fill out this form to renew an Alternative Teaching Certificate if you are employed by a county and enrolled in the county’s alternative certification

Fill out Form 55 if you are employed for a specific assignment in a county as a Professional Accountant, a Professional Business Official, or a School

The School Nurse Certificate may be issued to an individual who has been employed as a substitute or full-time School Nurse in a county public school

Fill out this form if you have been hired by a county as an Educational Sign Language Interpreter.

You are eligible for this license if you teach Adult (basic) Education coursework.
